Parental Release and Consent for Gymnastics or Tumbling Lessons at Canyon Gymnastics:

Just a reminder that ALL classes are pickup and drop off only.  We do take pictures and film once a month  for you to see what they are working on in class.   We try hard to keep distractions to zero to help the kids focus and practice without siblings and parents in the room. Please only sign up if you think your child is OK without you for 50 minutes.

* Make-up Classes- We do NOT offer make-up classes if your child misses a lesson. It has been difficult with so many kids getting sick with Covid to provide make ups to so many with a limited staff. Just make what you can!  We do not provide refunds for missed lessons. We still have to pay coaches to be there. Thanks for understanding. ***

Welcome to Canyon Gymnastics! While we make every effort to provide a fun and safe environment, it is important that every participant of Canyon Gymnastics and his/her legal guardian(s) understand that Gymnastics, like many sports, is dangerous by its very nature. Gymnastics could lead to bodily injuries, permanent disability, and even death.

In consideration of our child participating in Gymnastics activities:

• I/We acknowledge, agree, and represent that my/our child and I/us understand the nature of the gymnastics, including those risks explained to me/us above, and give consent for my/our child/children listed as students  to participate in Canyon Gymnastics activities.
• I/We release and forever discharge Canyon Gymnastics, its owners, employees, and legal representatives from any and every claim, demand, action or right of action, of whatever kind of nature arising from or by reason of any bodily injury or personal injuries known or unknown, death or property damage that may occur as a result of the minor / parents or any family members use of the facilities of Canyon Gymnastics, or any activities in connection with the use of those facilities and waive any rights I/We may have to bring such actions;
• I/We represent that my/our child is in good health and does not suffer from any illness, condition, or malady that would impair his/her ability to engage in gymnastics I/We have had the opportunity to consult with his /her physician prior to this signing.
• I/We release Canyon Gymnastics, its owners, employees, and legal representatives from any claim whatsoever on account of first aid, CPR, treatment, or service rendered or not rendered to me/us or my/our child during our or his/her use of the facilities of Canyon Gymnastics.
• I/We acknowledge that I/we have carefully read this release and know the contents of the same and sign this release as my/our own free act, and I/we have been given the opportunity to have an attorney review this release and explain its contents to me/us before I sign it.

*If you choose to stop in the middle of a season you will be responsible to sell your remaining classes because we held your spot and may have turned others away. There are exceptions if a child just cries through classes due to age. 🙂  If you need a class time change we can move you depending on availability. We recommend having your child watch the videos on our website under the CLASSES tab to see what classes are like and to see if they would be interested before signing up.
